BIG WAVE GROUP
VOLUNTEER AGREEMENT
Big Wave Group (BWG) is a 501(c)(3) nonprofit organization dedicated to providing housing, access to opportunities, and a sense of community for adults with special needs. The BWG campus includes the Big Wave Center at 386 Airport Street, Half Moon Bay, CA 94019, and the Big Wave Farm at 350 Airport Street, Half Moon Bay, CA 94019.
This agreement applies if you are a volunteer, parent, staff, resident, or the responsible party for a resident, at any activity or event being held on the Big Wave Center or the Big Wave Farm premises, which may include (but are not limited to) staffing, farming, building, recreation & fitness, enrichment activities, sporting activities, or any activity or job in the Big Wave kitchen. As a BWG volunteer, you agree that you are volunteering of your own will and expect no compensation in return.
Photography/Media Waiver: By signing this form, you are giving permission to BWG to use photos, videos, files, and audiotapes in which you appear, for publicity and funding purposes.
General Release of Liability: In consideration for permission to access, use, or volunteer at the Big Wave Center or Big Wave Farm premises, I (the undersigned) waive all claims against BWG and Big Wave Community Cooperative, Inc., together with each of their respective boards of directors, employees, officers, members, managers, investors, and volunteers, as the case may be (all of the foregoing, collectively, the BWG Parties), and hold the BWG Parties harmless from any and all claims, suits, losses, or related causes of action for damages, including but not limited to claims that may result from any injury or death, accident or otherwise, during or arising in any way from activities occurring in, on or about the Big Wave Center or Big Wave Farm premises. I further agree to indemnify and hold harmless the BWG Parties from any claims which I might make or which might be made on my behalf by others, or which might be made against me by others, arising from activities at the Big Wave Center or Big Wave Farm premises. This Agreement shall be governed by the laws of the State of California. This Agreement shall remain valid unless expressly revoked by the participant in writing, with receipt acknowledged by BWG.
Permission for Medical Treatment: In the event of a medical emergency while on the Big Wave Center or Big Wave Farm premises, I authorize BWG and the employees, officers, or volunteers of BWG to arrange for emergency medical care, treatment, and/or transportation (such as by ambulance) as necessary. In the event it is impossible to receive instruction from you or your emergency contact, consent is given to any licensed physician and/or surgeon to administer drugs and medication, and to perform such surgical treatment as he/she thinks the existing emergency requires for pain relief and/or preservation of your life, and/or health and well-being. This authorization and consent for treatment is given to BWG in conjunction with any authorized event.
Insurance: BW does not assume any responsibility for or obligation to provide me with financial or other assistance, including but not limited to medical, health, or disability benefits/insurance. I expressly waive any such claim for compensation or liability on the part of BWG beyond what BWG may freely offer in the event of injury or medical expenses incurred by me.
By signing this agreement, I express my understanding and intent to enter into this Waiver of Liability willingly and voluntarily.
